Stars Engrg Penalized Following 2021 Tuas Factory Catastrophe

A court in Singapore has levied a fine of S$500,000 on industrial firm Stars Engrg due to significant workplace safety failures associated with the tragic factory explosion in Tuas that occurred in February 2021. The firm's director, Chua Xing Da, received a sentence of 18 months and one week in prison after being found guilty of breaching workplace safety regulations and obstructing the investigation.

This explosion resulted in the deaths of three employees and left seven others with critical burn injuries.

Faulty Machine Operations Triggered Catastrophe

Investigations revealed that the blast was a result of a heated mixing machine being operated without adequate technical oversight. The machine had a history of overheating, oil leaks, and fire incidents prior to the fatal event, but operations continued under unsafe conditions.

Authorities determined that numerous avoidable errors were directly linked to the incident.

Adjudication on Severe Safety Breaches

The court proceedings revealed that Chua directly supervised the machine's installation and trained workers while approving changes without consulting the equipment's manufacturer. Despite numerous warning signs, machinery continued to be operated in unsafe conditions.

The judge concluded that there was a failure to establish needed maintenance and safety protocols.

Manager Found Guilty

Meanwhile, production manager Lwin Moe Tun received a six-week jail term after being convicted of obstructing justice by deleting relevant messages and photographs linked to the investigation, although he was acquitted of another charge related to workplace safety.

Investigators indicated that evidence was intentionally discarded after the explosion.

Ministry Enhances Safety Regulations

In response to the incident, Singapore's Ministry of Manpower indicated that the explosion could have been avoided with better risk management and equipment upkeep. As a result of the investigation, tougher workplace safety measures for high-risk machinery and combustible dust management will come into force in January 2025.

The ministry confirmed that all suggestions from the inquiry committee have been implemented.

Significance of Workplace Safety Emphasized

This catastrophic event remains one of the most serious industrial incidents in Singapore in recent history. Authorities emphasize that this case underscores the necessity for companies to adhere to safety regulations strictly and consult experts prior to modifying industrial machinery.

The court permitted the company to settle its fine in installments until July 2027, while the sentenced individuals are set to commence their prison terms in July.
